Visa for medical treatment
This means that you want to enter Germany for a short stay of up to 90 days in order to undergo medical treatment.
All required documents need to be submitted at the personal interview in 1 copy in DIN A4 format including the application form.
Please do not staple the photocopies.
Original certificates / documents must be presented separately. Your original documents will be returned to you once your application has been processed.
The following documents have to be submitted as indicated below:
completely filled out form in English or German, signed by the applicant himself
90 Euro
Payable in cash in NGN at the current exchange rate of the consulate or via POS (only possibe at the consulate).
Applications of persons who are married to a German are free of charge.
- not older than 6 month
- uniform white background
- size: 3.5x4,5 cm
- not glued or stapled to the application form
- original passport + copy of data page
- issued in the last 10 years
- valid for at least 6 more months and at least two free pages
if born after 13th December 1992:
- certified copy of the local birth register OR
- affidavit (“declaration of age” deposed to by one of the parents or the head of the family, if both parents are late)
- as proof of a legal residence status in Nigeria
- only for applicants with a nationality other than Nigerian
e.g.:
- hotel reservation
- invitation from a family meber/friend
e.g.:
- Bank statements for the last three months
- Pay slips for the last three months
- formal obligation (“Verpflichtungserklärung”) according to Sec. 66 to 68 of the German Residence Act covering the entire duration of stay, signed by a person living in Germany at the local competent office in Germany
Document must be presented in Original + one copy
☐ marriage certificate and birth certificates of dependent children, if applicable + 1 copy
☐ current personal bank statements of the last 3 months (preferably salary account) - Only bank statements issued and stamped by the bank are accepted
☐ proof of land ownership or ownership certificate, if applicable + 1 copy
☐ if employed: proof of employment from Nigeria

- salary statement for the last three months
- current salary account statement
- COI / COR / Certificate of Incorporation / Registration of the company in Nigeria
- FIRS / Federal Inland Revenue Service Income Tax Clearance Certificate / Company Income Tax Clearance Certificate of recent years
☐ if self-employed:
- business registration certificate
- proof of active business (bills of lading, import declaration forms, invoices) + 1 copy
- COI / COR / Certificate of Incorporation / Registration of the company in Nigeria, MEMO / Memorandum and Articles of Association / Company and Allied Matters Act
- FIRS / Federal Inland Revenue Service Income Tax Clearance Certificate / Company Income Tax Clearance Certificate of recent years
☐ for children and students: official leave letter from school or university, including proof of paid school fees and reports

a self-addressed prepaid courier envelope from any major courier service, addressed to yourself (please ensure the address on is legible to facilitate delivery)
The required documents must be submitted complete at the personal interview. Documents sent in advance cannot be considered.
It is not possible to process incomplete applications. Incomplete applications will be rejected.
